Size
Shape - edges, well demarcated? regular/irregular shape?
Surface -color and texture, scaly vs non-scaly (epidermis lesions are scaly)
Special features - fluid filled?, mobility, temperature/inflammation, other features.
Other terms
Abscess - large pus filled area
Ulcer - discontinuation of the epithelial lining into the epidermis or dermis
Fissure- linear discontinuation of the epithelial lining into the epidermis and dermis
Atrophy- thinning of the skin
Lichenification- thickening of the skin, leather like
Scale - shedding skin
Crust - Dried sebum and exudates
Erythematous - redness
Ecchyomosis - bruising
Excoriation - scratched
